Adult & Minor Liability Waiver and Release Agreement
(Includes COVID-19 Clause — Read Carefully Before Signing)

This agreement applies to all participants, whether adults or minors, who take part in any martial arts, fitness, or related activity at The Grind Martial Arts Academy LLC.
If the participant is under 18, a parent or legal guardian must sign below on the minor’s behalf.

1. Acknowledgment of Risk

I understand that participation in martial arts, fitness training, and related activities involves inherent risks, including but not limited to serious injury, illness, paralysis, or death.
These risks may result from my own actions, the actions of others, equipment failure, unsafe conditions, or negligence by The Grind Martial Arts Academy LLC, its owners, coaches, or staff.
I voluntarily assume all such risks for myself or my child.

2. Waiver and Release

In consideration of being allowed to participate, I (and on behalf of my minor child, if applicable) hereby release, waive, and discharge The Grind Martial Arts Academy LLC, its owners, instructors, employees, volunteers, landlords, and affiliates from any and all liability, claims, or causes of action for injury, illness, disability, death, or property damage arising out of participation — even if caused by their negligence.

3. Indemnification

I agree to indemnify and hold harmless The Grind Martial Arts Academy LLC and all Releasees from any and all claims, actions, damages, or expenses (including attorney’s fees) that may result from my participation or my child’s participation.

4. COVID-19 Acknowledgment

I understand that COVID-19 is contagious and that The Grind Martial Arts Academy LLC has implemented preventative measures but cannot guarantee protection.
By entering the facility or participating, I voluntarily assume all risk of exposure and release The Grind Martial Arts Academy LLC from any claims related to infection or illness arising before, during, or after participation.

5. Understanding and Agreement

I have read and understood this waiver, and I am signing voluntarily.
I understand this document affects my legal rights and that by signing, I am waiving the right to sue The Grind Martial Arts Academy LLC for negligence or any injury arising from participation.
